Job Description:

- Developing plans, Policies, SOPs & form/format for complete medication management process

- Anticipating, preventing and investigating medication error of all types- Prescription errors transcription errors, dispensing errors and administration errors

- Work closely with department of infection control for developing a robust antibiotic stewardship program.

- Maintaining established and implementing new quality Initiative required as per the JCI / NABH and other quality service requirements / protocol

- Monthly assessment, analysis and reporting of medication errors with suggestion/implementation on improvement parameters

- Keeping pace with current and upcoming MMU related issues with implementation of the changes as covered Reviewing/drafting and implementing changes required in the medication management plan (MMP).

- Adverse drug reaction monitoring, analysis and reporting

- Overseeing and coordinating prescription audit activities, including Transplant services.

- Answering all the queries generated from hospital staff related to proper dose/route/frequency of drugs, drug use, storage, stability, overdoses of drugs and other medication related issues

- Preventing drug interaction by reviewing the prescription and suggestions given to the concerned doctor and monitoring the same.

- Suggesting dose adjustments in case of abnormal Pharmacokinetics and Pharmacodynamics of drugs as per patient's clinical condition

- Suggesting doses adjustment of immunosuppressant's as per principals of therapeutic drug monitoring.

- Has basic knowledge of Transplantation of Human Organs and tissues Act, 1994 and concept of Brain Death diagnosis, certification and Ethical issues in organ donation and transplantation

- Provide patient education and counseling to the transplant patients and their families through all phases of transplant.

- Oversight and timely checking of the imprest stocks and crash carts including listing of drugs including in the respective areas

- Communicating and coordinating with clinicians in regarding to medication related issues

- Regular visits to the wards for cross checking of the medication management system including checking of patient's medical records

- Ensuring proper use, safety and storage of the high alert medication in various areas of the hospital

- Attending various quality related meeting including medical services meeting, infection control meeting etc, and providing necessary and useful inputs

- Monitoring the usage of Restricted antibiotic in the hospital

- As and when a new drug/molecule is being procured, check and comment for the quality related issues for the same

- Monitoring the new drug procured by pharmacy for their safety profile and providing necessary inputs and suggestion to add the new drugs to the formulary

- Arranging and coordinating drug therapeutic committee meetings within specified time frames with preparation of action plan and minutes and minutes accordingly

- Yearly review and updating the Hospital Formulary as per the recommendations by Drug and therapeutic committee

- Oversight of the Cytotoxic admixture area with time to time training session for the staff

- Active participating in the trainings and skill up gradation sessions for the staff.

- Oversight of drug storage and quality dispensing from the pharmacy

- Active involvement and prompt action for any JCI/MMU/Pharmacy quality related issues

- Participating in other hospital/pharmacy activities related to JCI and other quality practices with active inputs

- Responsible for maintaining the development of compliance with all human subject research policies and procedures

- Responsible for pharmacy staffs Job Skill and Competence assessment

- Ensure ethical conduct of clinical research

- Reviewing and monitoring the drug content of the hospital information system.

- Facilitate the development of quality indicators in relation to safe medication usage and pharmacy.

- Analyzing and interpreting data (often using specialist computer applications) making recommendations based on findings from internal audits.

- Performing internal audits and ensuring the compliance to JCI/NABH standards

- Conduct periodical training to doctors, nurses, pharmacist and paramedic staff (If required) to upgrade their knowledge base.

- Ensure safety and precautionary measures are in place to protect Clinical Pharmacists from exposure to toxic substances & infections while chemo admixture, chemo waste disposal & while in ward rounds.

PHARMACOVIGILANCE:

- To oversee the Apollo Pharmacovigilance center in reporting, collating, analyzing and Documenting Adverse Drug Reactions as per guidelines.

Preferred Candidate Profile:

- Behavioural - Leadership Skills, Conflict handling, Communication Skills, Crisis management, Passion for patient safety, act as a facilitator to team and other Healthcare professionals.

- Technical - Conceptual understanding of drugs, clinical pharmacy, Transplantation of Human organs and tissue Act & rules related therapeutic areas and soft skills in relation to Patient/ Customer / staff counselling.

- Strong personality with excellent communication and email writing skills.
