Home Care Nurse - Healthcare (0-5 yrs)
Job Summary:
We are seeking a compassionate and skilled Home Care Nurse to provide high-quality, individualized care to patients in their homes. The ideal candidate will work closely with patients, families, and healthcare professionals to monitor health status, administer medications, and support recovery and quality of life at home.
Key Responsibilities:
- Perform in-home assessments of patients- physical, emotional, and social needs.
- Administer medications and treatments as prescribed by a physician.
- Provide wound care, catheter care, and other skilled nursing interventions.
- Monitor vital signs, document patient conditions, and report changes to physicians.
- Educate patients and families about disease processes, treatments, and self-care techniques.
- Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
- Maintain accurate clinical documentation and comply with regulatory standards.
- Ensure infection control practices are followed.
- Support patients and families with end-of-life care when applicable.
- Both Freshers and Experienced are eligible.
