Job Title: Pharmacist

Job Summary:

The pharmacist is responsible for the preparation, dispensing, and management of medications in compliance with legal and regulatory requirements. They provide drug information and counseling to patients, collaborate with healthcare providers to optimize therapeutic outcomes, and monitor for potential drug interactions, side effects, and medication errors.

Key Responsibilities:

Dispensing Medications:

- Accurately dispense prescription medications as per physicians- orders.

- Check for accuracy and completeness of prescriptions and verify dosage, strength, and form of medication.

- Ensure proper labeling and packaging of medications.

Patient Counseling and Education:

- Provide patients with information regarding their prescriptions, including proper use, potential side effects, and drug interactions.

- Educate patients on health conditions and preventive care, including the importance of adhering to prescribed regimens.

- Advise patients on over-the-counter (OTC) medications, including their use and safety.

Medication Management:

- Review patient medication records for potential drug interactions, allergies, and contraindications.

- Monitor patient progress and advise healthcare providers on medication adjustments if necessary.

- Manage refills and prescription renewals within regulatory guidelines.

Collaboration with Healthcare Providers:

- Communicate with doctors, nurses, and other healthcare providers to ensure optimal medication therapy.

- Participate in multidisciplinary healthcare teams to discuss patient care plans and therapeutic outcomes.

Regulatory Compliance:

- Ensure that all medications are dispensed in accordance with local, state, and federal laws, including controlled substances.

- Maintain accurate and confidential patient records.

- Stay up-to-date on new drug information, policies, and regulations.

Inventory Management:

- Oversee inventory control, including ordering, stocking, and rotating medications.

- Manage expired medications and coordinate their safe disposal.

Compounding (if applicable):

- Prepare customized medications for patients (e.g., creams, ointments, or oral liquids) when required.
