Job Description :
1. Responsibilities include the following and other duties may be assigned
2. Explain the features of the Relex Medication Therapy Management Program to prospective or existing customers/patients
3. Effect medication replenishment, dosage tracking and obtain patient feedback.
4, Monitor outcomes and follow up on reminders to patient against counselling notes from Clinical Pharmacist and required lab tests
5. First level patient counselling and guidance on correct use of medication for achieving desired/targeted outcomes
Communicate customer feedback and customer experience on products/services and/or modifications to existing products/services to Operations, Marketing and Quality teams.
6. Support customer outreach programs through activities associated with theme of the program
Provide information and explanation of diagnoses, options, outcome and risk and escalate where required
7. Data updation in all tracking platforms on a daily basis
